About the Role

We are seeking a highly skilled and dedicated individual to join our team as a Tenancy Sustainment Coordinator. As a key member of our Mental Health and Complex Needs Service, you will play a crucial role in supporting individuals who are at risk of or experiencing homelessness.

Key Responsibilities
  • Deliver a tailored service offer to individuals who are at risk of or experiencing homelessness, focusing on developing and maintaining independent living skills and general wellbeing.
  • Work closely with referring agents, such as the H&F PATHS Team, to ensure effective and professional support.
  • Enable positive move-on to prevent rough sleeping, through the development and implementation of personalized support plans.
  • Co-produce needs, risk, and aspiration-based assessments with service users to ensure the delivery of a personalized service.
  • Assess and support service users with a range of needs, risks, and aspirations, including those with mental health and substance misuse issues, offender backgrounds, and complex life experiences.
  • Formulate, implement, and review SMART support plans to develop, review, and achieve goals and aspirations.
  • Engage service users' supporters in their support plans, including statutory and voluntary sector providers.
  • Utilize key-working sessions to enable service users to develop, sustain, and maximize independence, maintain their accommodation, and enjoy a good quality of life.
  • Deliver a flexible, dynamic, proactive, and responsive service according to client needs, including work outside of ordinary office hours.

Requirements

  • Strong work ethics.
  • Demonstrated knowledge and experience of working with housing and homelessness legislation, including the Homelessness Reduction Act and homelessness eviction and tenants' rights.
  • Demonstrated knowledge and experience of working with welfare benefits legislation, including welfare reform, and health and safety.
  • Demonstrated knowledge and experience of working with the causes of homelessness and the ability to create interventions with service users to address these.
  • Excellent customer service skills.
  • Impeccable organizational skills.
