Position Overview

An exceptional opportunity has emerged for a dedicated professional to join Norfolk Community Health and Care as a Patient Flow Coordinator. This role involves supporting the management of 28 Intermediate Care Beds across multiple care facilities. You will collaborate with a multidisciplinary team, engaging with families, patients, and caregivers to ensure efficient admissions and discharges from the care settings.

The ideal candidate will integrate into an existing team of Patient Flow Coordinators who currently assist various inpatient units. You will report to the Operations Manager and play a vital role within a compact team, working closely with medical staff, therapists, social workers, and care home managers.

Your responsibilities will also extend to the local university hospital, where you will facilitate the transfer of designated patients into Intermediate Care Beds, overseeing the entire process from admission through to discharge.

This position is permanent, offering a full-time schedule of hours per week across five days, from Monday to Friday, with no weekend or holiday shifts.

Key Responsibilities

Your duties will include:

  • Making onward referrals to suitable services.
  • Communicating effectively with patients and their families.
  • Proactively planning for patient discharge from the moment of admission.
  • Collaborating within a multidisciplinary team, including nurses, doctors, and therapists.
  • Facilitating problem-solving and leading weekly meetings.
  • Managing admissions and discharges from care facilities while ensuring precise record-keeping.

Job Responsibilities

On a daily basis, you will either be stationed at the local university hospital or one of the care facilities to assist with admissions and discharges. You will lead weekly multidisciplinary meetings for all care homes and follow up on any actions identified, which may involve liaising with families and other professionals, completing referral forms, communicating with patients, and coordinating transport.

It is essential to prioritize tasks effectively and work independently while also being a collaborative team member. Strong communication skills and organizational abilities are crucial.

Qualifications

Essential:

  • Higher level of education (NVQ level 3 or equivalent).

Desirable:

  • Evidence of degree-level education.
  • RSA III.
  • 'A' levels or equivalent in Mathematics and English.

Experience

Essential:

  • Relevant experience in primary care, the NHS, or social care.
  • Demonstrated ability to prioritize work systematically and manage pressure to meet deadlines.
  • Administrative experience related to service users or patients.
  • Proven experience working as part of a team.
  • Experience in day-to-day problem-solving related to patients and service users.

Desirable:

  • Experience utilizing enhanced communication skills.
  • Advanced problem-solving experience.

Skills/Knowledge

Essential:

  • Excellent planning, organization, and communication skills.
  • Understanding of relevant health and social care legislation.
  • Proficient in Microsoft Office, particularly Word and Excel.
