About the Role:

We are seeking a skilled Paediatric Complex Care Nurse to join our team at Thornbury Community Services. As a Paediatric Complex Care Nurse, you will play a vital role in delivering high-quality care and support to young people with complex needs in their own homes.

Key Responsibilities:

  • Provide bespoke care and support to young people with complex needs, enabling them to stabilise at home and achieve personal goals.
  • Work closely with families and other healthcare professionals to ensure seamless care and support.
  • Develop and implement individualised care plans to meet the unique needs of each client.
  • Provide education and training to families and carers on complex care needs and management.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support.

Requirements:

  • Minimum of 12 months' UK experience in the last two years working with people with clinical complex needs.
  • Tracheostomy care and suctioning, tracheostomy tube change, ventilation via tracheostomy, and ventilation via a mask skills are essential.
  • Availability for last-minute shifts on an as-and-when basis.
  • Willingness to travel and work in a community setting.
