Job Title: Senior Community Dementia Nurse Specialist

Job Summary:

We are seeking a highly skilled and experienced Senior Community Dementia Nurse Specialist to join our team at South West Yorkshire Partnership NHS Foundation Trust. The successful candidate will work as part of a multi-disciplinary team to provide high-quality clinical assessments, care, and treatment for individuals experiencing dementia and their families.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals with dementia, including the use of relevant psychometric assessments.
  • Develop and implement individualized care plans, including therapeutic interventions and support for carers.
  • Work collaboratively with the multi-disciplinary team to provide coordinated care and support.
  • Provide education and support to individuals with dementia and their families, promoting independence and empowerment.
  • Contribute to the development and implementation of service policies and procedures.
  • Participate in quality improvement initiatives and contribute to the evaluation of service effectiveness.

Requirements:

  • Registered Nurse (Mental Health or General Nursing) with relevant experience in working with older adults and individuals with cognitive impairment.
  • Independent Non-Medical Prescriber qualification or willingness to undertake training.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to work.
  • Strong problem-solving and analytical skills, with the ability to prioritize and manage a caseload.

Desirable Qualifications and Experience:

  • Postgraduate qualification in a relevant field, such as dementia care or mental health.
  • Experience of working in a community mental health setting, with a focus on dementia care.
  • Knowledge of dementia assessment tools and therapeutic interventions.
  • Experience of working with carers and families, providing education and support.
