Job Title: Senior Community Dementia Nurse Specialist
Job Summary:
We are seeking a highly skilled and experienced Senior Community Dementia Nurse Specialist to join our team at South West Yorkshire Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality clinical assessments, care, and treatment to individuals experiencing dementia and their families.
Key Responsibilities:
- Conduct comprehensive assessments of individuals with dementia, including the use of relevant psychometric assessments.
- Develop and implement individualized care plans, incorporating therapeutic interventions and support for individuals and their families.
- Provide ongoing care coordination, including the management of referrals, provision of clinical supervision, and facilitation of case review discussions.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and coordination.
- Participate in the evaluation of the efficacy of anti-dementia medication and provide recommendations for treatment.
- Act as a care coordinator for individuals with dementia, ensuring their needs are met and their families are supported.
- Develop and maintain effective relationships with individuals, families, and healthcare professionals to ensure comprehensive care delivery.
Requirements:
- RN Mental Health or RN General nursing with relevant experience in working with older people and/or individuals with cognitive impairment.
- Independent Non-Medical Prescriber qualification or willingness to undertake and complete within an agreed timeframe.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and healthcare professionals.
- Strong problem-solving and organizational skills, with the ability to prioritize and manage multiple tasks and responsibilities.
- Ability to work independently and as part of a multidisciplinary team, with a flexible attitude to work and a willingness to adapt to changing circumstances.
