We are pleased to announce an opportunity for a skilled nurse or allied health professional to join our supportive community neurological rehabilitation team as a Neurological Navigator.
Role PurposeThis position is focused on enhancing the journey of individuals with complex, long-term neurological conditions as they navigate through the healthcare and community support systems.
Key Responsibilities- Provide navigation and support for patients diagnosed with rare or rapidly progressing neurological disorders.
- Conduct specialized initial assessments and coordinate information, support services, and guidance for patients from diagnosis to end-of-life care.
- Collaborate as an integral member of the community neuro team.
We offer a comprehensive in-service training program, regular clinical and managerial supervision, team meetings, and a dedicated multidisciplinary team (MDT). Opportunities for funded training to enhance clinical or leadership skills are also available.
QualificationsWe are seeking a candidate with extensive experience in various specialist neurology and neurorehabilitation environments. Applicants from related disciplines such as Occupational Therapy, Physiotherapy, Speech and Language Therapy, Psychology, Nursing, or Social Work are encouraged to apply.
Skills and AttributesThe ideal candidate will possess:
- Excellent communication skills and a flexible approach.
- The ability to work independently while maintaining close collaboration with colleagues.
HCPC or NMC registration is mandatory for this role. A full valid driving license and access to a vehicle for business purposes are also essential, unless exempt under the Equality Act.
