About the Role

We are seeking a highly skilled and experienced Adult Community Neurological Physiotherapist to join our team at Hertfordshire Community NHS Trust. As a key member of our Community Neuro Service, you will be responsible for managing a caseload of patients with complex neurological conditions, providing expert advice and guidance to healthcare professionals, carers, and patients.

Key Responsibilities
  • Conduct comprehensive assessments of patients with diverse or complex presentations/multi-pathologies using advanced clinical reasoning skills and manual assessment techniques.
  • Develop and implement individualized treatment plans for patients with stroke and other complex neurological conditions.
  • Assess and manage clinical risk within your patient caseload.
  • Represent the physiotherapy service and/or individual patients at multi-disciplinary team meetings.
  • Ensure the delivery of a coordinated multidisciplinary service and integrate physiotherapy treatment into the treatment programme.
Person Specification
  • Physiotherapy degree or equivalent.
  • HPC registration.
  • Experience of working with neurological patients in a community or inpatient setting at Band 5 level.
  • Documented evidence of professional development and reflective practice in relevant aspects of neurological rehabilitation.
Desirable Qualifications and Experience
  • Attendance at relevant post-graduate neurological training courses.
  • Experience of working in a neurological rehabilitation setting at Band 6 level.
