Job Title: Band 5/6 Community Rehab Physiotherapist

This is an exciting opportunity to join our team as a Band 5/6 Community Rehab Physiotherapist at Hertfordshire Community NHS Trust. As a key member of our Neurological Rehabilitation Team, you will be responsible for managing a caseload of patients with complex neurological conditions, providing expert advice and guidance to healthcare professionals, carers, and relatives.

Key Responsibilities:
  • Conduct comprehensive assessments of patients with diverse or complex presentations/multi-pathologies, using advanced clinical reasoning skills and manual assessment techniques to provide accurate diagnoses.
  • Develop and implement individualized physiotherapy treatment plans, incorporating evidence-informed practice and treatment options.
  • Represent the physiotherapy service and individual patients at multi-disciplinary team meetings, ensuring effective communication and collaboration with healthcare professionals.
  • Supervise and coordinate more junior staff, students, and rehabilitation assistants, promoting a culture of excellence and continuous learning.
  • Participate in health promotion and health education initiatives, promoting active and healthy lifestyles.
Requirements:
  • Physiotherapy degree or equivalent.
  • HPC registration.
  • Experience working with neurological patients in a community or inpatient setting at Band 5 level.
  • Documented evidence of professional development and reflective practice in relevant aspects of neurological rehabilitation.
Desirable:
  • Attendance at relevant post-graduate neurological training courses.
  • Experience working in a neurological rehabilitation setting at Band 6 level.
