About the Role

We are seeking a highly skilled and experienced Adult Community Neurological Physiotherapist to join our team at Hertfordshire Community NHS Trust. As a key member of our Community Neuro Team, you will be responsible for managing a caseload of patients with complex neurological conditions, providing expert advice and guidance to healthcare professionals, carers, and patients.

Key Responsibilities
  • Conduct comprehensive assessments of patients with diverse or complex presentations, using advanced clinical reasoning skills and manual assessment techniques.
  • Develop and implement individualized treatment plans, incorporating a range of therapeutic interventions, including manual therapy, exercise, and education.
  • Work collaboratively with multidisciplinary teams to ensure seamless care coordination and effective discharge planning.
  • Provide expert advice and guidance to healthcare professionals, carers, and patients, promoting optimal outcomes and quality of life.
  • Participate in regular supervision and peer review to ensure high-quality practice and ongoing professional development.
Requirements
  • Physiotherapy degree or equivalent.
  • HPC registration.
  • Experience working with neurological patients in a community or inpatient setting at Band 5 level.
  • Documented evidence of professional development and reflective practice in relevant aspects of neurological rehabilitation.
Desirable Qualifications and Experience
  • Attendance at relevant post-graduate neurological training courses.
  • Experience working in a neurological rehabilitation setting at Band 6 level.
