Job Summary

We are seeking a skilled and dedicated Community Staff Nurse to join our team at Buckinghamshire Healthcare NHS Trust. As a Community Staff Nurse, you will play a vital role in providing high-quality nursing care to patients in their homes, promoting independence and preventing hospital admissions.

About the Role

This is an exciting opportunity for a motivated and compassionate nurse to work in a dynamic and supportive team environment. You will be responsible for assessing patients' physical and psycho-social needs, delivering evidence-based interventions, and collaborating with healthcare assistants and clinicians to achieve optimal outcomes.

Main Responsibilities
  • Contribute to service and quality improvement initiatives
  • Assess and deliver therapeutic interventions to patients with a range of medical conditions
  • Manage a caseload, delivering and evaluating care to patients with varying needs
  • Participate in referral management and common assessment processes
  • Contribute to the development of clinical pathways and protocols
