About the Role

We are seeking a highly skilled and motivated District Nurse/Community Case Manager Nurse to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our Community Health Care Team, you will play a vital role in delivering high-quality patient care and support in the community setting.

Key Responsibilities
  • Provide specialist clinical assessment, analysis, planning, delivery, and evaluation of patient care
  • Develop and maintain effective partnerships with patients, carers, and other healthcare professionals
  • Contribute to the development and implementation of care plans that meet the needs of patients with complex conditions
  • Work collaboratively with the multidisciplinary team to ensure seamless care transitions and effective discharge planning
  • Participate in quality improvement initiatives and contribute to the development of best practices in community care
Requirements
  • Registered Allied Healthcare Professional/Registered Level 1 Nurse
  • Degree/Diploma in Health/Nursing Studies or related subject
  • Evidence of recent Continuous Professional Development
  • Relevant postgraduate clinical training in areas such as elderly care, long-term conditions, respiratory disease, cardiac disease, dementia, rehabilitation, trauma and orthopaedics, and end-of-life care
