About the RoleThis is an exciting opportunity for a skilled and dedicated nurse to work in a fast-paced community setting, providing comprehensive care to patients in their own homes. You will be responsible for assessing, planning, and delivering care to patients with a range of complex needs, working closely with other healthcare professionals to ensure seamless care pathways.
Key Responsibilities- Assess and plan care for patients with complex needs, including those with long-term conditions, disabilities, and mental health issues.
- Deliver high-quality care to patients in their own homes, working independently and as part of a multidisciplinary team.
- Develop and maintain partnerships with patients, families, and carers to ensure person-centered care.
- Contribute to the development of community health services, including service redesign and quality improvement initiatives.
- Participate in regular multidisciplinary team meetings to ensure effective communication and collaboration.
- Registered Allied Healthcare Professional/Registered Level 1 Nurse.
- Degree/Diploma in Health/Nursing Studies or related subject.
- Evidence of recent Continuous Professional Development.
- Relevant postgraduate clinical training in areas such as elderly care, long-term conditions, and rehabilitation.
