District Nurse/Community Case Manager

About the Role
We are seeking a highly skilled and motivated District Nurse/Community Case Manager to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our Community Health Care Team, you will play a vital role in delivering high-quality patient care and supporting the health and wellbeing of our local community.
Key Responsibilities
- Provide expert clinical assessment, analysis, planning, delivery, and evaluation of patient care
- Develop and maintain effective partnerships with patients, carers, and other healthcare professionals
- Contribute to the development and implementation of care plans that meet the individual needs of patients
- Work collaboratively with the multidisciplinary team to ensure seamless care transitions and effective discharge planning
- Participate in quality improvement initiatives and contribute to the development of best practice guidelines
Requirements
- Registered Allied Healthcare Professional/Registered Level 1 Nurse
- Degree/Diploma in Health/Nursing Studies or related subject
- Evidence of recent Continuous Professional Development
- Relevant postgraduate clinical training in Elderly, Long Term Conditions, Respiratory Disease, Cardiac Disease, Dementia, Rehabilitation, Trauma and Orthopaedics, and End of Life
