Community Nurse

We are seeking a skilled and dedicated Community Nurse to join our team at Buckinghamshire Healthcare NHS Trust. As a Community Nurse, you will play a vital role in providing high-quality patient care and support in the community.
Main Responsibilities- Contribute to the development and implementation of care plans to meet the needs of patients with a variety of medical conditions.
- Assess the physical and psycho-social needs of patients and deliver therapeutic interventions based on best available evidence to improve health outcomes.
- Manage a caseload of patients, delivering and evaluating service delivery to ensure high-quality care.
- Work collaboratively with other healthcare professionals to ensure seamless care delivery and patient outcomes.
- Contribute to the development of protocols and procedures for clinical pathways.

We are looking for a candidate with:
- Registered Nurse or student nurse awaiting confirmation of NMC registration.
- Post-registration education and extended skills, such as intravenous drug administration and compression dressings.
- Demonstrated experience in relevant clinical areas, including care of the elderly, community, orthopaedics, neurology, respiratory, and palliative care.
- Knowledge of working with single assessment, planning, and delivery of care.
- Experience of electronic clinical systems and technology, such as RIO.

- Communicate effectively with patients, relatives, carers, and other professionals.
- Work under pressure and manage a caseload of patients.
We require a candidate who is:
- Able to travel to other locations within the trust.
