Job Title: Paediatric Community Nurse

We are seeking a highly skilled and compassionate Paediatric Community Nurse to join our team at Thornbury Community Services. As a Paediatric Community Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs, enabling them to achieve their personal goals and live fulfilling lives.

Key Responsibilities:
  • Provide expert care and support to young people with complex needs, including those with Cornelia De Lange syndrome, ASD, and Bilateral profound sensorineural hearing loss.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
  • Conduct regular assessments and reviews to ensure the highest standards of care are maintained.
  • Communicate effectively with young people, families, and other healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing professional development to stay up-to-date with best practices and evidence-based care.
Requirements:
  • Minimum 6 months of experience in the last 2 years in a Paediatric Community Nursing role.
  • Recent experience in Tracheostomy Care & Suction, Tracheostomy Tube Change, Ventilation via Tracheostomy, Humidification, Care & use of Gastrostomy, and Changing Gastrostomy Buttons.
  • 12 months minimum UK-based experience.
