We are seeking a skilled and dedicated Community Nurse to join our team at Buckinghamshire Healthcare NHS Trust. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their homes, promoting independence and preventing hospital admissions.
The successful candidate will have experience in community nursing and be able to work effectively with healthcare assistants and clinicians to deliver nursing interventions. You will also be responsible for assessing patients' physical and psycho-social needs, developing and implementing care plans, and evaluating service delivery.

Main Responsibilities:
- Contribute to service and quality improvement processes
- Assess patients' needs and deliver therapeutic interventions
- Manage own caseload and deliver care to patients with various medical conditions

- Contribute to referral management and ensure patients receive appropriate care
