About the Role:

  • Engage young people who have substance use issues and provide them with advice, support, and structured interventions to promote their health and wellbeing needs.
  • Work with clients between the ages of 18 and 25 to support their transition from young people's services to adult services.
  • Liaise with various agencies involved in the young adult's broader care plan to ensure comprehensive support.
  • Conduct thorough assessments and develop individual care plans that incorporate integrated care and transition support.
  • Promote young adults' and young people's participation and co-production opportunities to identify potential gaps in provision and unmet needs.
  • Collaborate with the 16+ supported accommodation Multi-Disciplinary Team to provide a coordinated, flexible, and responsive approach to support.

About You:

  • Knowledge and experience of providing interventions for service users affected by substance use.
  • Excellent communication and IT skills, with the ability to work independently and manage time effectively.
  • Proactive and able to work on own initiative, with a good working knowledge of mental health interventions, services, and good practice.
  • Strong interpersonal skills, with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.
  • Ability to manage change successfully, prioritizing the needs of service users.
  • Understanding of information governance processes and commitment to follow and apply necessary safeguards.
  • Commitment to ongoing learning and development, with a focus on improving and broadening professional knowledge and skills.
