Young Persons Transitional Worker

About the Role:
- Engage young people who have substance use issues and provide them with advice, support, and structured interventions to promote their health and wellbeing needs.
- Work with clients between the ages of 18 and 25 to support their transition from young people's services to adult services.
- Liaise with various agencies involved in the young adult's broader care plan to ensure comprehensive support.
- Conduct thorough assessments and develop individual care plans that incorporate integrated care and transition support.
- Promote young adults' and young people's participation and co-production opportunities to identify potential gaps in provision and unmet needs.
- Collaborate with the 16+ supported accommodation Multi-Disciplinary Team to provide a coordinated, flexible, and responsive approach to support.
About You:
- Knowledge and experience of providing interventions for service users affected by substance use.
- Excellent communication and IT skills, with the ability to work independently and manage time effectively.
- Proactive and able to work on own initiative, with a good working knowledge of mental health interventions, services, and good practice.
- Strong interpersonal skills, with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.
- Ability to manage change successfully, prioritizing the needs of service users.
- Understanding of information governance processes and commitment to follow and apply necessary safeguards.
- Commitment to ongoing learning and development, with a focus on improving and broadening professional knowledge and skills.
