Exciting Opportunity for a Critical Care Outreach Practitioner

An exceptional opportunity has emerged for a proactive and innovative Band 6 Critical Care Nurse to become a vital member of our Critical Care Outreach team.


We seek a self-driven, versatile, and knowledgeable registered nurse who demonstrates clinical confidence both independently and collaboratively within a team setting.


The ideal candidate will possess outstanding communication abilities, strong decision-making skills, and clinical proficiency, along with a commitment to enhancing healthcare delivery.


The Critical Care Outreach Team operates within the Heart and Lung Centre and provides a continuous service across the Trust, supporting approximately 840 beds.


We are in search of a dedicated and motivated nurse with critical care experience, committed to upholding high standards of safe and dignified patient care.


The successful applicant will integrate into a specialized nursing team and will be expected to work flexibly to ensure 24-hour care.


There will be opportunities to advance assessment and clinical skills through a competency-based framework within an educational pathway.


Excellent communication skills are essential to foster an effective multidisciplinary approach to the recognition, escalation, and management of deteriorating patients across the Trust.


We take pride in the educational support provided in both ward and classroom settings, maintaining strong networking relationships regionally and nationally with other specialist teams.


A specialized interest in altered airway management would be advantageous; however, any relevant experience will also be appreciated.

Key Responsibilities:

  • Ensure safe and effective clinical practice.

1.1 Clinical Leadership and Team Collaboration

Practice within an ethical framework that prioritizes the well-being and safety of patients.


Be accountable for assessing care needs, developing personalized care plans, and implementing and evaluating care autonomously.


Provide specialized nursing care for high-risk patients within the hospital, including Level 3 transfers to other clinical areas or facilities.

1.2 Evidence-Based Practice

Ensure that care planning and delivery align with professional values of care, compassion, and respect, acting as a change agent to develop clinically effective practices.

1.3 Continuous Quality Improvement

Support the Senior Sister/Charge Nurse in fostering a culture that protects patients from harm while ensuring a safe and effective service with ongoing quality enhancement.

1.4 Patient Safety

Promote a clean and safe environment for all by adhering to relevant legislation, policies, and protocols, including health and safety and clinical governance.


Ensure high standards of care and record-keeping in compliance with professional councils and local standards.


Assess risks associated with patient care and implement practices to mitigate those risks, including the safe use of medical devices.


Monitor care standards and take appropriate actions when they fall below expected levels.


Report incidents and near misses promptly and effectively, collaborating with the Senior Sister/Charge Nurse to minimize future risks.


Contribute to the development and implementation of evidence-based protocols, standards, policies, and clinical guidelines.


Enhancing Patient Experience:

  • Coordinating the Patient Journey

Ensure an efficient patient journey by planning and coordinating care episodes, promoting safe transitions, and optimizing communication with interdisciplinary teams.


Foster a culture of continuous quality improvement through audits, patient feedback, and reflective practice.


Clinical Expertise:
Advise on clinical decisions and monitor the quality of care provided, utilizing expert clinical knowledge relevant to your practice area.


Act as a patient advocate, ensuring safe and harm-free care is prioritized in every decision.


Be a visible presence within wards, maintaining regular communication with patients and staff to promote transparency and a caring environment.


Identify opportunities for service development by supporting the Outreach Senior Sister/Charge Nurse in gathering patient and carer feedback.


Ensure that all individuals within your care sphere are treated with dignity and respect, understanding their unique needs.


Team Management and Development:

Act as a visible role model, adhering to expected standards of behavior and promoting a supportive ethos to empower staff.


Lead by example, influencing care delivery in an innovative and responsive manner.


Support the appraisal process by ensuring personal development plans align with team and organizational objectives.


Establish robust communication networks within the team and work effectively with colleagues, patients, and their families.


Take reasonable care of health and safety for yourself and others, cooperating with employers to ensure compliance with health and safety regulations.
