Position Overview

We are seeking a highly skilled and knowledgeable Senior Clinical Practitioner to become a vital member of our Adult Community Services team. This role requires the ability to operate independently, making informed decisions to address health requirements as part of a multidisciplinary team.

The focus of our team is to transition towards a model led by Advanced Clinical Practitioners, moving away from traditional medical frameworks. This approach aims to proactively manage patients who are still receiving care in a hospital setting, enhancing their health status to facilitate a swift return to their homes. The role includes conducting ward rounds, participating in team huddles, and executing both scheduled and emergency clinical interventions to avert unnecessary transfers back to acute care facilities. Additionally, there is a chance to shape the escalation protocols that will ensure a rapid response from the Community Urgent Care team, thereby preventing emergency department visits or hospital admissions, while also contributing to the ongoing assessment of this innovative operational model.

This position is crucial in maintaining the vibrancy of our community hospital, which provides specialized rehabilitation services across the region. The successful candidate will collaborate closely with the management team and clinical staff to achieve our shared objectives.

The individual in this role will be expected to demonstrate a high level of professional autonomy and make critical decisions that meet the expectations and demands of the position. Responsibilities include:

  • Supporting the Lead Advanced Clinical Practitioner in establishing a more efficient, patient-centered service.
  • Ensuring the safe referral and discharge processes for patients.
  • Providing direct, compassionate care to patients, families, and communities.
  • Maintaining accountability for professional practice standards as set by the relevant regulatory body.

Qualifications

Essential:

  • NMC Registration as a Nurse or HCPC registration.
  • Master's level education in a healthcare-related field or working towards it.
  • Health Assessment qualification.
  • Experience as a V300 Non-Medical Prescriber.

Desirable:

  • Experience in autonomous practice within a secondary care setting.
  • Evidence of professional development within the last year specific to the area of practice.
