About the Role

We are seeking a highly motivated and proactive Senior Advanced Nurse Practitioner/Frailty Lead to join our team at The Royal Wolverhampton NHS Trust. As a key member of our multidisciplinary team, you will play a crucial role in shaping the future of Frailty Care in our new Ambulatory Emergency Care Unit.

Key Responsibilities
  • Deliver advanced level practice, encompassing aspects of education, research, and management, while maintaining a strong focus on direct, compassionate care provision.
  • Promote public health and well-being through 'making every contact count' as an approach to change behavior.
  • Lead effective communication with the multidisciplinary team regarding patient care and assess risks involved in patient care, ensuring the utilization of practices and protocols to minimize those risks.
  • Monitor standards of care and take appropriate actions when standards fall below expected levels.
  • Report incidents and near misses promptly and appropriately, taking effective action to minimize future risk.
  • Identify and implement systems to promote contribution of advanced level nursing and demonstrate the impact of advanced level nursing to the healthcare team and the wider health and social care sector.
Person Specification
  • Masters degree
  • Independent prescribing qualification/specialist training
  • NMC registration/professional registration
  • Relevant postgraduate specialist qualification
  • Evidence of professional updating within the last 12 months
  • Elderly care experience
  • Community experience
