Position Overview
We are seeking a skilled and knowledgeable Senior Clinical Practitioner to become a vital member of our Adult Community Services team. This role requires the ability to operate independently, making informed decisions to address health requirements as part of a multidisciplinary team.
The Senior Clinical Practitioner will play a crucial role in managing patients who are still receiving care within a hospital environment, working to enhance their health status to facilitate a swift transition to their home environment. Responsibilities will include leading ward rounds, participating in team huddles, and executing both scheduled and emergency clinical interventions to avert unnecessary returns to acute care settings.
This position is essential in maintaining the vibrancy of our community hospital, which specializes in rehabilitation services across the region. The successful candidate will collaborate closely with the management and clinical teams to achieve our service delivery goals.
Key Responsibilities
The individual in this role will be expected to demonstrate a high level of professional autonomy and make critical decisions that meet the expectations of the service. Responsibilities include:
- Supporting the medical team in innovating service delivery that prioritizes efficiency and patient-centered care.
- Ensuring the safe and effective management of patient referrals and discharges.
- Providing compassionate care directly to patients, families, and communities.

Qualifications, Skills, and Experience
Essential:
- NMC Registration as a Nurse or HCPC registration.
- Master's level education in a healthcare-related field or working towards it.
- Health Assessment qualification.
- Experience as a V300 Non-Medical Prescriber.
Desirable:
- Experience in autonomous practice within a secondary care setting.
- Evidence of professional development within the last year relevant to the area of practice.
- Knowledge of rehabilitation and frailty patient care.
- Experience in an acute care environment.
