Pharmacist in Medicines Optimisation

We are seeking a highly skilled and experienced Senior Clinical Pharmacist to join our team at King's College Hospital NHS Foundation Trust. As a key member of our medicines optimisation team, you will play a vital role in improving patient outcomes and reducing healthcare costs.
Key Responsibilities- Develop and implement policies, guidelines, and procedures to improve pharmacy services and patient care.
- Provide clinical pharmacy services to patients, including medication reviews, discharge planning, and patient education.
- Work closely with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Contribute to the development and implementation of evidence-based prescribing guidelines and policies.
- Collaborate with general practice staff to promote and embed the role of a GP practice pharmacist.
- Work within the eHub to process medication eConsults, reconcile medications, and process medication workflow across the Hurley Group primary care sites.
- Provide operational and clinical support to medicines optimisation developments and prescribing initiatives.
- Develop and deliver training programs to promote medicines optimisation and high-quality prescribing practices.
- Registered Pharmacist with 1 year of competency-assessed and examined professional registration with the General Pharmaceutical Council.
- Postgraduate Certificate in Pharmacy Practice or equivalent qualification.
- Desirable: Registered Independent Prescriber or willingness to complete.
