We are seeking a highly skilled Clinical Pharmacist to join our team at North Thames Genomics Laboratory Hub. As a Clinical Pharmacist, you will play a crucial role in optimising patient care by ensuring that patients receive the best possible use of their medicines.
Key Responsibilities- Patient-Facing Clinics: Conduct patient-facing clinics for patients with long-term conditions, where medicine optimisation is required. Deliver pharmacy-related aspects of the Network DES/LES, such as conducting structured medication reviews.
- Medicine Optimisation: Review the ongoing need for each medicine, monitoring needs, and support patients with their medicines, ensuring they get the best use of their medicines. Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.
- Medicine Management: Manage medicines at discharge from hospital, reconcile medicines, and identify and rectify unexplained changes. Work with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
- Quality Improvement: Identify cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. Implement changes to medicines that result from MHRA alerts, product withdrawal, and other local and national guidance.
- Education and Training: Provide education and training to the primary healthcare team on therapeutics and medicines optimisation.
- CQC Compliance: Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.
- Masters degree in pharmacy (MPharm) or equivalent
- Completed 18-month CPPE pathway or equivalent
- Independent prescriber or actively working towards
- Prior GP/clinical pharmacy experience
- Competent with specialist in-depth knowledge for multiple chronic disease areas, with the ability to consult and independently prescribe
