Pharmacist - Medicines Optimisation Specialist

We are seeking a highly motivated and experienced pharmacist to join our team as a Resident Pharmacist - Medicines Optimisation. This is an exciting opportunity to develop and use your medicines optimisation knowledge and skills to enhance the patient experience and improve flow across the Trust.
Key Responsibilities- Be a role model and actively promote safe, evidence-based prescribing practices.
- Support and promote medicines optimisation services in your designated specialty.
- Engage with the wider healthcare team to ensure patients are discharged in a timely and safe manner.
- Provide education and training for undergraduate healthcare professionals.
- Be actively involved in audit and research.
- Deliver metrics that demonstrate the benefit of this role in providing pharmaceutical services to patients.

We are the regional specialist care provider for South-West London, Surrey, and beyond. Our hospital provides a wide range of services, including emergency care, surgery, and specialist care for conditions such as cancer, stroke, and trauma.
Person SpecificationWe are looking for a pharmacist with the following skills and experience:
- Computer literacy.
- Good interpersonal skills.
- Capacity to work effectively under pressure.
- Logical approach to problem-solving.
- Experience of handling conflict effectively.
Desirable skills and experience include:
- Familiarity with JAC computer systems and electronic prescribing systems.
- Experience of management and/or leadership.
We require a pharmacist with the following qualifications and training:
- Masters Degree in Pharmacy.
- Membership of the General Pharmaceutical Council.
- Evidence of Continuing Professional Development (CPD).
Desirable qualifications and training include:
- Certificate in General Pharmacy Practice or equivalent.
