Job Summary

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at King's College Hospital NHS Foundation Trust. As a Band 7 Pharmacist Specialist, you will play a key role in the development and implementation of medicines optimisation strategies across our Trust.

Main Responsibilities
  • Medicines Optimisation: Work with our multidisciplinary team to deliver high-quality, cost-effective, patient-focused pharmaceutical care to patients.
  • Interface and Discharge Liaison: Provide support to wards and units within the Care Group, ensuring seamless communication and coordination of patient care.
  • Clinical Leadership: Lead on the provision of clinical pharmacy services to sub-specialties within the Care Group, ensuring a high level of clinical pharmacy input.
  • Staff Supervision: Act as a clinical role model, supervising rotational staff at ward level and promoting excellence in clinical practice.
  • Medicines Usage Audit: Conduct audits to promote cost-effective prescribing and optimise medication use across the Care Group.
  • eConsults and Medication Workflow: Work within the eHub to process medication eConsults, reconcile medications, and manage medication workflow across primary care sites.
  • Operational and Clinical Support: Provide operational and clinical support to medicines optimisation developments and prescribing initiatives, aligning with Trust and service objectives.
  • Training and Education: Deliver training on medication-related issues and policies/guidelines to general practice staff, promoting medicines optimisation and high-quality prescribing.
  • Electronic Discharge Information: Collate and feedback on electronic discharge information to improve the hospital computerised discharge letter and communication process.
Person Specification
  • Qualifications: Registered Pharmacist with 1 year of competency-assessed and examined professional registration with the General Pharmaceutical Council.
  • Postgraduate Qualifications: Postgraduate Certificate in Pharmacy Practice or equivalent qualification.
  • Prescribing Qualifications: Registered Independent Prescriber or willingness to complete.
  • Experience: Previous specialist clinical experience in a relevant area.
  • UK Professional Registration: Must have appropriate UK professional registration.
