We are seeking a highly skilled Clinical Pharmacist to join our team at North Thames Genomics Laboratory Hub. As a Clinical Pharmacist, you will play a key role in leading our medicines optimisation efforts, working closely with our healthcare team to ensure patients receive the best possible care.
Key Responsibilities- Medicines Optimisation: Lead the development and implementation of medicines optimisation strategies to improve patient outcomes and reduce healthcare costs.
- Clinical Leadership: Provide clinical leadership and guidance to our healthcare team on medicines-related matters, ensuring that our patients receive high-quality care.
- Medicines Management: Develop and implement effective medicines management systems to ensure that patients receive the right medicines, at the right time, and in the right dose.
- Collaboration and Communication: Work closely with our healthcare team, including doctors, nurses, and other healthcare professionals, to ensure seamless communication and collaboration on medicines-related matters.
- Quality Improvement: Identify areas for quality improvement and implement changes to our medicines management systems to ensure that our patients receive the best possible care.
- Master's degree in pharmacy (MPharm) or equivalent
- Completed 18-month CPPE pathway or equivalent
- Independent prescriber or actively working towards
- Prior GP/clinical pharmacy experience
- Competent with specialist in-depth knowledge for multiple chronic disease areas, with the ability to consult and independently prescribe
