Job Title: Palliative Care Clinical Nurse Specialist – Community Team

We are seeking a highly skilled and compassionate Palliative Care Clinical Nurse Specialist to join our Community Team at Woking & Sam Beare Hospices. As a key member of our team, you will provide specialist care and support to patients and their families in the community, working closely with healthcare professionals to ensure seamless care coordination.

Key Responsibilities:
  • Provide specialist knowledge and expertise in caring for patients with life-limiting illnesses
  • Assess, plan, implement, and evaluate patient care, ensuring optimal coordination and delivery of care
  • Develop and maintain effective relationships with the multi-disciplinary team to ensure collaborative working for the benefit of patient care
  • Deliver high standards of care directly and assist in the management of junior staff
Requirements:
  • Registered Nurse with relevant post-registration courses and palliative care qualification
  • Independent Nurse Prescriber and management/leadership course
  • Mentorship/teaching qualification and UK Registration
  • Disclosure and Barring Service Check
