Associate Clinical Nurse Specialist - Community Palliative Care

We are seeking a motivated and enthusiastic Nurse to join our community specialist palliative care team at Woking & Sam Beare Hospices. The team delivers and continues to develop palliative care services for people facing progressive and life-limiting conditions across our catchment area.

About the Role

The successful candidate will have the opportunity to work closely with our multidisciplinary team, including Clinical Nurse Specialists, Associate Clinical Nurse Specialists, Paramedics, Registered Nurses, Healthcare Assistants, and Specialist Doctors. The team provides comprehensive care to patients and their families, and the successful candidate will be expected to contribute to this effort.

Key Responsibilities

  • Deliver high-quality palliative care to patients and their families
  • Work collaboratively with the multidisciplinary team to develop and implement care plans
  • Contribute to the development and implementation of new services and programs
  • Participate in education and training opportunities to enhance skills and knowledge

Requirements

  • UK Registration as a Nurse
  • Experience in palliative care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
