About the Role

We are seeking a dedicated and compassionate Care and Support Worker to join our team at Metropolitan Housing Trust. As a Care and Support Worker, you will play a vital role in providing high-quality care and support to our service users, ensuring their physical, emotional, and social needs are met.

Key Responsibilities
  • Work collaboratively with the Care and Support Manager to develop and implement individual care plans that meet the unique needs of our service users.
  • Provide person-centered care and support, promoting independence, dignity, and well-being.
  • Conduct risk assessments and manual handling assessments to ensure a safe working environment.
  • Monitor and review care plans, making adjustments as necessary to ensure the best possible outcomes for our service users.
  • Communicate effectively with service users, their families, and other stakeholders to ensure seamless care and support.
  • Participate in handover meetings, staff meetings, and training sessions to stay up-to-date with best practices and organizational policies.
Requirements
  • Experience of working with vulnerable adults, preferably in a care and support setting.
  • Understanding of the complexities of care and support, including health and safety policies and procedures.
  • Ability to work flexibly, including evenings, weekends, and public holidays.
  • Basic IT literacy and ability to use electronic recording systems.
  • Excellent communication and interpersonal skills.
