Ambulance Paramedic

We are seeking an experienced and enthusiastic Ambulance Paramedic to join our Community Independence Service (CIS) team in Hammersmith & Fulham. As a key member of our team, you will play a vital role in delivering high-quality patient care and making a positive impact on the lives of our patients.
Key Responsibilities- Provide community-based responsive input to patients with undifferentiated, varied, and complex needs, ensuring holistic assessment and community care pathway planning.
- Work across the Rapid Response Team and Home First Team (H1st) to provide clinical expertise and support.
- Develop and implement effective pathways of care through hospital to community, building relationships with hospital and community partners.
- Contribute to the development of the service within the CIS, achieving excellent results in admission avoidance and reducing the length of hospital stay for the people of Hammersmith and Fulham.

Requirements- Appropriate professional registration (e.g. NMC, Adult Nursing/HCPC Paramedic).
- Experience of working in a multidisciplinary team and managing acute episodes of care.
- Knowledge of current NHS policy initiatives and experience of leading audit or project.
- Ability to manage complaints in a sensitive and appropriate manner, providing excellent customer service.
