About the Role

We are seeking an experienced and skilled Ambulance Paramedic to join our team at West London NHS Trust. As a key member of our Community Independence Service, you will play a vital role in delivering high-quality patient care and making a positive impact on the lives of our patients.

Key Responsibilities
  • Provide community-based responsive input to patients with undifferentiated, varied, and complex needs, ensuring holistic assessment and community care pathway planning.
  • Work across the Rapid Response Team and Home First Team, providing clinical expertise and support to patients and families.
  • Develop and implement effective pathways of care, working closely with hospital and community partners to ensure seamless transitions and optimal patient outcomes.
  • Contribute to the development of the service, identifying opportunities for improvement and implementing changes to enhance patient care and experience.
Requirements
  • Appropriate professional registration (e.g. NMC, Adult Nursing/HCPC Paramedic)
  • Experience of working in a multidisciplinary team and managing complex patient needs
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • Ability to work independently and as part of a team, with a flexible approach to working hours and shifts
  • Full driving license and ability to drive a car across the H&F borough
