About the Role:The successful candidate will be responsible for providing community-based responsive input to patients with undifferentiated, varied, and complex needs. This will involve conducting holistic assessments and developing community care pathways to prevent unnecessary acute hospital admissions.
You will work closely with our hospital and community partners to ensure seamless transitions of care and provide evidence-based practice to patients from a variety of acute/hospital and community settings.
Key Responsibilities:- Conduct holistic assessments and develop community care pathways to prevent unnecessary acute hospital admissions
- Provide community-based responsive input to patients with undifferentiated, varied, and complex needs
- Work closely with hospital and community partners to ensure seamless transitions of care
- Deliver high-quality evidence-based practice to patients from a variety of acute/hospital and community settings
To be successful in this role, you will need to be an experienced Registered General Nurse or Paramedic with a strong background in community health and social care. You will also need to be able to work independently and as part of a team, with excellent communication and interpersonal skills.
Car driving is essential for this post, and you will be required to work across the tri-borough sites on an 8 am – 8 pm or 10 am -10 pm shift pattern.
