Job Title: Community Paramedic Practitioner

About the Role:

We are seeking a highly skilled and experienced Community Paramedic Practitioner to join our team at K & W Healthcare. As a Community Paramedic Practitioner, you will play a vital role in providing high-quality patient care and support to our patients in their own homes and community settings.

Key Responsibilities:

  • Assess, diagnose, and manage patients with a range of conditions, including acute and chronic illnesses.
  • Provide holistic care and support to patients, including medication management, wound care, and palliative care.
  • Work collaboratively with other healthcare professionals, including GPs, nurses, and social workers, to provide comprehensive patient care.
  • Develop and implement care plans that meet the individual needs of patients, including those with complex needs.
  • Provide education and support to patients and their families to empower them to manage their conditions effectively.
  • Contribute to the development and implementation of clinical guidelines and policies to improve patient care.
  • Participate in quality improvement initiatives to enhance patient outcomes and experience.

Requirements:

  • Registered Paramedic qualification and current registration with the Health and Care Professionals Council.
  • Minimum of 2 years' experience as a Paramedic, with a strong background in community care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong commitment to patient-centered care.
  • Knowledge of relevant legislation, policies, and guidelines that impact patient care.
