About the Role

We are seeking a dedicated and experienced Transitional Liaison Mental Health Practitioner to join our team. In this role, you will play a vital part in supporting male patients within the Criminal Justice System as they transition back into the community.

Key Responsibilities

  • Provide comprehensive mental health assessments and interventions for male patients within a forensic setting.
  • Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and probation officers, to develop individualized care plans.
  • Implement relapse prevention strategies and crisis intervention techniques to support patient well-being.
  • Conduct risk assessments and contribute to the safety and security of patients and staff.
  • Maintain accurate and up-to-date records of patient care and progress.

Essential Criteria

  • Proven experience working at Band 5 or above within a forensic setting.
  • A strong understanding of the health needs specific to males within the Criminal Justice System.
  • Demonstrated ability to work autonomously and as part of a multidisciplinary team.
  • Experience in formulating risk assessments and implementing safety protocols.
  • Excellent communication, interpersonal, and organizational skills.

Desirable Criteria

  • Experience working in a controlled environment.
  • Fluency in Welsh.
