Job Overview

A unique opportunity has arisen within the Offender Personality Disorder Pathway Team (OPDP) at Swansea Bay University Health Board. This role is a partnership with HM Prison & Probation Service (HMPPS) and involves working with high-risk males transitioning from custody to the community.

Main Duties

As a Transitional Liaison Mental Health Practitioner, you will be responsible for building relationships with prison and community teams, probation staff, and third sector organizations to facilitate the smooth transition of high-risk males. This will involve identifying treatment needs and referring to local teams in Wales, while providing relational support during the transition period.

You will work within a developing, highly specialist, and supportive multidisciplinary team, contributing to ongoing service development and receiving ongoing support via managerial supervision and performance review.

Key Responsibilities
  • Build relationships with prison and community teams, probation staff, and third sector organizations
  • Identify treatment needs and refer to local teams in Wales
  • Provide relational support during the transition period
  • Contribute to ongoing service development
  • Work within a multidisciplinary team
Requirements

To be successful in this role, you will need:

  • Registered Mental Health Nurse or equivalent professional qualification
  • Post-registration study to degree level or evidence of working towards same or equivalent experience
  • Experience of working at Band 5 or above within a forensic setting
  • An understanding of the health needs of males within the Criminal Justice System
  • An understanding of working collaboratively with male patients and all interested parties to develop relapse prevention and crisis intervention strategies
  • The ability to work autonomously and as part of a multidisciplinary team
  • The ability to work in a psychological manner or have experience of working with Trauma
  • Able to formulate risk assessments
  • Ability to work in a controlled environment
  • Ability to speak Welsh
