Job Overview

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our Offender Personality Disorder Pathway Team (OPDP) within Swansea Bay University Health Board. This is a unique opportunity to work with a multidisciplinary team to support individuals with complex mental health needs within the Criminal Justice System.

Main Duties

As a Transitional Liaison Mental Health Practitioner, you will be responsible for building relationships with prison and community teams, probation staff, and third sector organizations to facilitate the smooth transition of high-risk individuals from custody to the community. You will work collaboratively to identify treatment needs, develop relapse prevention strategies, and provide psychological support to individuals during their transition.

Key Responsibilities
  • Work with a caseload of high-risk individuals to identify treatment needs and develop personalized care plans
  • Collaborate with multidisciplinary teams to provide psychological support and interventions
  • Develop and maintain relationships with prison and community teams, probation staff, and third sector organizations
  • Contribute to ongoing service development and improvement initiatives
  • Participate in training and professional development opportunities to enhance skills and knowledge
Requirements

To be successful in this role, you will need:

  • Registered Mental Health Nurse or equivalent professional qualification
  • Post-registration study to degree level or evidence of working towards same or equivalent experience
  • Experience of working at Band 5 or above within a forensic setting
  • Understanding of the health needs of males within the Criminal Justice System
  • Ability to work autonomously and as part of a multidisciplinary team
  • Excellent communication and interpersonal skills
