Dental Nurse Specialist

Job Overview
We are seeking a highly skilled and compassionate Forensic Mental Health Practitioner to join our team at Swansea Bay University Health Board. As a Transitional Liaison Mental Health Professional, you will play a crucial role in supporting high-risk males as they transition from custody to the community.
Main Duties of the Job
Working within the Offender Personality Disorder Pathway Team (OPDP), you will be responsible for building relationships and engaging with Prison and Community Teams, Probation staff, and third sector organisations to facilitate the timely and smooth transition of high-risk males. This will involve identifying treatment needs and referring to local teams in Wales, while providing relational support during the transition period.
Key Responsibilities
- Develop and implement relapse prevention and crisis intervention strategies in collaboration with male patients and interested parties.
- Conduct risk assessments and formulate plans to ensure the safety and well-being of service users.
- Work autonomously to fulfill the role of Transitional Liaison Mental Health Practitioner, while also contributing to ongoing service development and improvement.
- Provide psychological support and interventions to service users, including those with trauma and complex mental health needs.
Requirements
- Registered Mental Health Nurse or equivalent professional qualification.
- Post-registration study to degree level or evidence of working towards same or equivalent experience.
- Experience of working at Band 5 or above within a forensic setting.
- Ability to work autonomously and as part of a multidisciplinary team.
- Excellent communication and interpersonal skills.
