Job Summary

The post-holder will provide high-quality care and advice to patients with mental health problems, working independently to assess and support individuals with medium-term mental health needs. The post-holder will work closely with GPs, nurses, and other professionals to develop and support a clear understanding of mental health needs within the population.

Main Responsibilities

The successful applicant will be responsible for managing a caseload of patients with primary care mental health needs, working closely with Secondary Care Mental Health Services, Local Primary Mental Health Support Services, and Third Sector Organizations to ensure cohesive pathways of care.

Person Specification
