Job Overview

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our Offender Personality Disorder Pathway Team. As a Transitional Liaison Mental Health Practitioner, you will play a crucial role in supporting high-risk men as they transition from custody to the community.

Main Duties

Working within a multidisciplinary team, you will be responsible for building relationships and engaging with prison and community teams, probation staff, and third sector organizations to facilitate the timely and smooth transition of high-risk men. This will involve identifying treatment needs, referring to local teams, and providing relational support during the transition period.

Key Responsibilities

  • Develop and implement relapse prevention and crisis intervention strategies in collaboration with male patients and interested parties.
  • Conduct risk assessments and formulate effective plans to ensure the safety and well-being of service users.
  • Work autonomously and as part of a team to provide high-quality care and support to high-risk men.
  • Participate in ongoing service development and contribute to the improvement of services for male patients.

Requirements

  • Registered Mental Health Nurse or equivalent professional qualification.
  • Post-registration study to degree level or evidence of working towards same or equivalent experience.
  • Experience of working at Band 5 or above within a forensic setting.
  • Ability to work in a psychological manner or have experience of working with trauma.
  • Ability to formulate risk assessments and work in a controlled environment.
