Job Overview

This is a unique opportunity to join the Offender Personality Disorder Pathway Team within Swansea Bay University Health Board.

As a Transitional Liaison Mental Health Practitioner, you will work closely with prison and community teams, probation staff, and third sector organisations to facilitate the smooth transition of high-risk men from custody to the community.

The post holder will be responsible for building relationships, engaging with service users, and developing relapse prevention and crisis intervention strategies.

With a focus on improving men's experience during transition and reducing reoffending, this role requires a compassionate individual who can work autonomously and as part of a multidisciplinary team.

Main Duties of the Job

Key responsibilities include:

  • Working within the Offender Personality Disorder Pathway Team to facilitate the transition of high-risk men from custody to the community
  • Building relationships with prison and community teams, probation staff, and third sector organisations
  • Developing relapse prevention and crisis intervention strategies for service users
  • Working autonomously and as part of a multidisciplinary team to improve men's experience during transition

Person Specification

Essential criteria include:

  • Registered Mental Health Nurse or equivalent professional qualification
  • Post-registration study to degree level or evidence of working towards same or equivalent experience

Desirable criteria include:

  • A commitment to continuous professional development and evidenced-based practice

Knowledge and Experience

Essential criteria include:

  • Experience of working at Band 5 or above within a forensic setting
  • An understanding of the health needs of males within the Criminal Justice System
  • An understanding of working collaboratively with male patients and all interested parties to develop relapse prevention and crisis intervention strategies

Desirable criteria include:

  • Ability to speak Welsh

  • Ability to travel within large geographical area and to work hours flexibly
