About the Role

The Norfolk & Suffolk Foundation NHS Trust is seeking a skilled Mental Health Practitioner to join our Community Mental Health Team. As a key member of our team, you will be responsible for providing assessment, treatment planning, and interventions to service users with moderate to severe mental illness.

Main Responsibilities

  • Assess new referrals and work with a caseload of service users to develop and implement effective treatment plans.
  • Contribute to the wider team in areas such as FACT and Duty roles.
  • Work collaboratively with other healthcare professionals to provide comprehensive care to service users.

Requirements

  • Current Professional qualification as a Registered Nurse, Occupational Therapist, or Social Worker.
  • Experience in mental health practice, including risk assessment and risk management.
  • Competent assessment skills and knowledge of risk assessment.
  • Ability to time manage and reflect on clinical practice.
