About the Role

We are seeking a compassionate and dedicated Support Worker to join our team. In this role, you will play a vital part in empowering individuals with complex needs to live fulfilling lives. You will provide personalized support, assisting them with daily living activities, promoting their independence, and fostering their well-being.

Key Responsibilities

  • Provide practical assistance with daily tasks such as personal care, meal preparation, and medication management.
  • Encourage social interaction and participation in community activities.
  • Develop and implement individualized support plans to meet the unique needs of each person.
  • Maintain accurate records and documentation.
  • Work collaboratively with other professionals, including therapists, case managers, and family members.
