About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our Crisis Resolution Home Treatment Team in Slough. As a key member of our team, you will play a vital role in delivering high-quality care to our patients and providing an alternative to traditional mental health admissions.

Key Responsibilities
  • Conduct thorough assessments and develop individualized care plans for patients with complex mental health needs.
  • Deliver evidence-based treatments and interventions to support patients' recovery and well-being.
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery and effective risk management.
  • Develop and maintain strong relationships with patients, families, and carers to promote engagement and empowerment.
Requirements
  • Registered Mental Health Nurse or Social Worker with a degree in health or social care.
  • Substantial experience of managing a busy caseload and working effectively in a multi-disciplinary team environment.
  • Excellent communication and interpersonal skills, with the ability to empathize and support patients with complex needs.
  • Ability to work flexibly and travel independently between locations to fulfill the requirements of the position.
