Job Overview

We are seeking a highly skilled and motivated individual to join our team as a Senior Mental Health Practitioner. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to patients in their own homes.

Main Responsibilities
  • Provide clinical and therapeutic care to patients, adhering to trust policies and protocols.
  • Develop, implement, and evaluate nursing plans in response to individual needs, in partnership with clients and carers.
  • Work autonomously, referring to senior colleagues as necessary, and maintain excellent communication with patients, relatives, and multidisciplinary teams.
  • Support and teach patients to self-manage their condition, empowering carers to actively participate in treatment plans.
  • Manage End of Life care, working in partnership with health, social, and voluntary agencies to ensure patients are treated with dignity and respect.
Requirements
  • Qualified Nursing Associate or Assistant Practitioner.
  • Car driver with a valid UK license.
  • Positive and motivational attitude.
