Job Summary

We are seeking a highly skilled and experienced Senior Practitioner to join our team in the Emergency Department, specializing in End of Life Care. This is an exciting opportunity to make a meaningful difference in the lives of patients and their families.

Key Responsibilities
  • Provide expert advice and support to patients, families, and healthcare professionals in the Emergency Department and community settings.
  • Develop and implement strategies to improve End of Life Care services, ensuring seamless transitions and optimal patient outcomes.
  • Collaborate with multidisciplinary teams to promote best practices, education, and research in End of Life Care.
  • Support the development of remote professional advice and peripatetic services to enhance patient care and reduce hospital admissions.
  • Contribute to the design and implementation of innovative solutions to improve patient experience and quality of care.
Requirements
  • Postgraduate qualification in a relevant field, with a strong background in End of Life Care.
  • Proven experience in clinical leadership, education, and research in End of Life Care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to remain calm and composed in high-pressure situations, with a strong commitment to patient-centered care.
  • Knowledge of local and national End of Life Care standards, policies, and guidelines.
