NHS Senior Practitioner

We are seeking a highly skilled and experienced Senior Practitioner to join our team at University Hospitals Plymouth NHS Trust. As a key member of our End-of-Life Care team, you will play a vital role in providing expert advice and support to patients, families, and healthcare professionals.
Key Responsibilities- Provide specialist in-reach support to community hospitals, including remote and onsite support to End-of-Life bed bases.
- Work collaboratively with multidisciplinary teams to develop and implement effective care plans for patients at the end of life.
- Conduct educational sessions to promote contemporary evidence-based knowledge and skills in end-of-life care.
- Support and facilitate best outcomes for patients and families, enabling discharge to preferred places of care.
- Coordinate and deliver remote advice and support telephone lines for professionals.
- Current registration with the NMC/HCPC etc UK registration.
- Post-qualification learning or equivalent master's level proven knowledge/specialist demonstrable experience in an appropriate area.
- Post-graduate specialist qualification/recent study or demonstrable experience relating to end-of-life care.
- Evidence of professional development relevant to the specialty.
