NHS Senior Practitioner

We are seeking a highly skilled and experienced Senior Practitioner to join our team at University Hospitals Plymouth NHS Trust. As a key member of our End of Life Care team, you will play a vital role in providing expert advice and support to patients, families, and healthcare professionals.
Key Responsibilities- Provide specialist in-reach support to community hospitals, including remote and onsite support to End of Life bed bases.
- Develop and implement strategies to improve patient care and experience, working closely with multidisciplinary teams.
- Collaborate with healthcare professionals to provide enhanced and streamlined End of Life care, including discharge planning and coordination.
- Support and promote an integrated approach to quality, ensuring that patient needs are met and exceeded.
- Provide education and training to healthcare professionals on contemporary evidence-based knowledge and skills in End of Life care.
- Current registration with the NMC/HCPC etc UK registration.
- Post-qualification learning or equivalent master's level proven knowledge/specialist demonstrable experience in an appropriate area.
- Post-graduate specialist qualification/recent study or demonstrable experience relating to End-of-Life care.
- Evidence of professional development relevant to the specialty.
