Job Summary

NHS Scotland is seeking an experienced pharmacist to lead the delivery of a new high-risk medicines monitoring service. The successful candidate will work in the primary care pharmacy team to lead on shared care of high-risk medicines and interface with secondary care specialist teams to ensure best care is delivered to patients.

Key Responsibilities

  • Provide expert advice on medicines to patients and healthcare professionals to ensure cost-effective and patient-centered care.
  • Lead on shared care of high-risk medicines in primary care and interface with secondary care specialist teams.
  • Develop and implement strategies to improve medicines safety and reduce harm.
  • Collaborate with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.

Requirements

  • Relevant experience in primary and/or secondary care with a strong clinical background.
  • Independent prescribing qualification.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
