The successful candidate will be instrumental in helping us achieve these goals by conducting medication histories, engaging with service users and their families regarding medication management, planning for patient discharge, utilizing near-patient dispensing, and collaborating with primary care for seamless transitions post-discharge.

Main Responsibilities
The successful applicant will collaborate with specialty pharmacists and all members of the multidisciplinary team (MDT) to coordinate all facets of the patient's medication journey, ensuring effective communication for a safe and integrated transfer of care.
Key responsibilities include:
- Assisting clinical teams with the procurement, storage, and management of medications within inpatient settings.
- Completing medication history and reconciliation upon patient admission.
- Providing information to patients and caregivers about their medications to promote adherence and understanding.
- Working closely with clinical teams to facilitate timely patient leave and discharge, employing near-patient dispensing processes to ensure timely medication delivery.
- Supporting the transfer of care into primary care settings, utilizing the DMS service and liaising with other healthcare professionals.
- Participating in the accuracy checking rota within the dispensary.
- Assisting in the training and development of trainees.
- Participating in weekend and bank holiday rotas.
