About the Role

We are seeking a highly skilled and experienced Senior Physiotherapist to join our team at East Suffolk and North Essex NHS Foundation Trust. As a Senior Physiotherapist - Care Home Project, you will play a key role in shaping a new role to meet the needs of Care Home residents, Care Homes, and commissioners, and deliver positive change.

Key Responsibilities
  • Facilitate effective, appropriate, and timely therapy support to Care Homes and their residents, providing education and clinical support in the community, avoiding attendance to the acute trust where possible.
  • Work as part of a Care Home project team, including nurses and therapy, to collectively support groups of Care Homes, while also working with the local Integrated Neighbourhood Team to ensure joined-up, seamless care.
  • Maintain open communication with social care providers, primary care, and commissioning to make recommendations for improvements to support effective outcomes for adults.
  • Develop and gather information to demonstrate the effectiveness of the Care Home project, including the production of regular monitoring reports.
Requirements
  • Proven post-registration experience in a health or social care setting.
  • Range of rotational acute/community care experiences, including acute discharge planning.
  • Demonstrable in-service training and reflective learning in these specialties.
  • Ability to work as an autonomous practitioner.
  • Experience of working with assistants/technicians.
  • Ability to build effective working relationships with patients/carers/colleagues.
  • Directing patient care.
Desirable Qualifications and Skills
  • Experience of evidencing and auditing practice.
  • Supervision of students in any environment.
  • Membership of SIG.
  • Completed rotation in Emergency Care (A&E, EAU, FAB) and/or Completed rotation in Complex and/or Completed rotation in Community care.
